Submitting the "Send a Request" form sends your details to the clinic so it can get in touch with you. It does not create a confirmed appointment, and it does not, by itself, establish a doctor-patient relationship. That relationship — and any appointment — begins only once the clinic contacts you directly and you agree on a scheduled visit.
